About this policy
This policy describes this website and its appointment-contact form. It is separate from the clinic’s Notice of Privacy Practices for medical records and patient care. Ask the clinic for its current HIPAA notice; agreeing to website terms does not waive your health-privacy rights.
Information you choose to provide
The form asks for a phone number, email address or both, your contact preference, and permission to contact you. A message of up to 300 characters is optional. Do not include symptoms, medical history, date of birth, insurance information, payment details, links or attachments. A request is not a confirmed appointment.
How contact requests are handled
Submitting sends your details through Cloudflare Workers and Google’s Gmail service to the clinic’s designated email inbox. The clinic may configure a separate copy to another authorized clinic inbox. After the email service accepts the primary notification, the receipt identifies the primary inbox used; acceptance does not guarantee inbox delivery or mean the clinic has read it. The notification includes a request reference, receipt time, language, source page and the policy version accepted. The clinic uses it to respond and arrange a visit. The website does not create an appointment, patient account or marketing subscription, and it does not queue failed requests. If delivery cannot be confirmed, call the clinic before submitting again.
Website technology and outside services
Cloudflare hosts the site and may process technical connection data such as IP address, browser information and security events. Turnstile checks submissions for abuse. Google processes notification emails for delivery through the clinic’s authorized sending account. The map loads from Google; following social or external links takes you to those providers. Their privacy practices also apply. The site saves a text-size preference in your browser. Its current application does not include advertising pixels or store form details in localStorage.
Access, retention and security
Delivery providers process information to operate the service, and clinic staff receive the message. The application does not create a separate request database or log form bodies, but email messages and provider records may remain under their retention settings. Contact the clinic about access, corrections, deletion or retention; legal and clinical recordkeeping duties may limit deletion. No website or ordinary email service can promise absolute security. Your choices and policy changes
You can call instead of using the form and can ask the clinic to stop following up about a request. A parent or authorized adult should contact the clinic for a child; do not enter a child’s health details here. Updated website policies will appear on these pages. Cookies, browser settings and provider policies
You can clear saved site preferences in your browser. Blocking scripts, cookies or embedded content may prevent maps or security checks from working; calling remains an alternative. Cloudflare website analytics
The published Pages site also uses Cloudflare Web Analytics for page-view and performance measurements. This can include page and referrer addresses, browser/device information and timing data. Cloudflare states that its usage analytics does not use cookies or localStorage. Our form code does not send contact details or message content to analytics. Health-privacy questions
A website contact request can still be sensitive even without a diagnosis. Do not use this page as authorization to disclose medical information. Ask the clinic for its current Notice of Privacy Practices, its privacy contact and an appropriate way to request records or raise a concern.
